Well, I think I have those moments on a daily basis... always picking up the door that is locked when I have a choice; keep buying the same damn book or cd thinking how much I've wanted it, but forgetting I already bought it, 3 times! You know, little things like that!

that's actually more inline with what Major Murphy (seconded by the USAF to Hughes aircraft during rocket sled tests), a USAF engineer, said about designing components:
If you design something that somebody can screw it up, someday someone will. In his case it was a connector on the sled that could be plugged in backwards...resulting in the sled speeding out of control with one of the original 7 astronauts on board...
